The patient, who had previously unde rgone mitral valve replacement with B ioprosthetic valve in 2013, and a recen techo confirmed degeneration of that prosthesis with severe narrowing and leak along with very high right heart p ressure (severe pulmonary hyperten sion) A second operation to replace th is will be prohibitively higher risk, hen ce he was planned to have elective non -surgical Transcatheter Mitral ViV rep placement in end of November, howev er, his journey took an unexpected tur n when he suffered a fall at home, lead ding to a left leg fracture neck of femu r (broken hip bone).
The urgency of his hip condition and t he critical nature of his heart ailment presented a daunting challenge. It was a high risk procedure that required a multidisciplinary approach and the on ly safer way to treat him is to do both procedures under the same general an aesthesia, first the Transcatheter Mitr al ViV followed by Left Total Hip Repla cement, in an Hybrid Cath Lab Operati ng Room. This is available in very few centres in India, and given the availab bility of the Hybrid Cath Lab OR, Cardi ac and Orthopedic expertise, this patie nt underwent Hybrid procedure safely and was mobilized within 36 hours.
“The patient underwent a transcathe ter mitral valve-in-valve procedure th rough groin access, a minimally invas ive technique that immediately reduce d his right-side pressure from 130 to 60 and a normally functioning mitral valv e. With his heart condition stabilized, the orthopedic team seamlessly procee ded with the hip replacement surgery on the same operating table at the Hy brid Operating Room,” said Dr R Anant haraman , Senior Interventional Cardi ologist Kauvery Hospital Chennai.
The procedures were a resounding suc cess. The patient was extubated from the ventilator and mobilized within 36 hours, defying all odds. His swift recov ery enabled him to return to his villag e and resume his active lifestyle.
